Utilization here is billable hours over hours logged. Non-billable time is still real time, which is why it stays on the sheet rather than being left off.
Day, what the time went to, how long, and whether it is billable. Anything more gets skipped by the person filling it in, and a timesheet nobody completes is worse than none because it looks like data.
Only if you pay for them or bill for them. Otherwise record worked hours; adding unpaid breaks as rows makes reconciliation with payroll harder for no benefit.
Shorten the form and move it to where the work already is. A week reconstructed on Friday is a guess, and the error lands squarely on the billable split, which is the number you price from.
