Fixed and verified are different states on purpose. An item the trade says is done but nobody has re-inspected is the one that reopens on handover day.

What is the difference between a punch list and a snag list?

Nothing but geography. Punch list is the American term, snag list the British one, and both mean the outstanding items between substantial completion and sign-off.

When should the punch list be created?

At the walkthrough before handover, and it should be closed before final payment is released. Starting it earlier tends to blur it into the issue log, which is a different list about problems during the work rather than defects at the end.

Why track the trade on each item?

Because the list gets actioned by sorting it: one electrician fixing eleven sockets in one visit is cheap, the same eleven items scattered across three visits is not.
